Forms for Enrollment
Pre-Enrollment Form-use this form to apply for enrollment and to secure a spot for your child at The Nursery at Our Lady of Sorrows School
Developmental History-This form is to be filled out just prior to your child's first day
Emergency Contacts-This form is to be returned as part of the enrollment packet
Emergency Medical Authorization-This form is to be returned as part of the enrollment packet
Universal Health Form-This form must be filled out by Child's Doctor and returned as part of enrollment packet
Authorized Pick Up Form-This form is to be returned as part of the enrollment packet
Allergy Information-This form is to be returned as part of the enrollment packet
Contact Information Release Form-This form is to be returned as part of the enrollment packet
Permission to Photograph-This form specifies where your child's photographs may be used. This form is to be returned as part of the enrollment packet.
Diaper Cream Authorization-This form is to be returned as part of the enrollment packet and any time there is a change in creams or schedule of application.
Sunscreen/ Insect Repellent Authorization-This form is to filled out every year in April and any time the brand of sunscreen or insect repellent changes.
Medication Administration Authorization-This form is to be filled out for every medication that needs to be administered while your child is at The Nursery. Make sure to take a copy with you to every Doctor appointment.
Authorization to Use Microwave Oven- This form allows the staff of The Nursery to use a microwave oven to reheat your child's food.
Handbook Sign Off- This form is to be signed after reading the Family Handbook. A link to the Handbook is located to the right.
Webcam Information-this form gives instructions for logging on. Return this form with the enrollment packet. Fill in your choice for user name and password filled out.
Closing Schedule-a list of the days The Nursery is closed during the year.
2010-2011 Calendar-a calendar of special events and center closings for the 2010-2011 year.
Potty Training Plan-Parents and Caregivers complete this form together when it is time to start potty training your child.
